Overview

All first report of injuries or illnesses should be submitted within 24 hours through the Origami Portal to ensure compliance for timely submission.

Eligibility

All employees whose names appear on the payroll of the University are covered under the program at no personal expense. This coverage includes student and wage employees.

Resources

• Contaminated Sharps Injury Reporting Form:  This form must be completed by all faculty, staff, and students who experience a contaminated sharps injury.
• HR Incident Guide:  Guide for Employee Relations to verify and submit incidents to WCI for claims processing.
• Notice to Employees of Workers Compensation Insurance:  Informs employees that Workers' Compensation Insurance coverage is provided in accordance with Chapter 502 of the Texas Labor Code.
• Notice of Injured Employee Rights and Responsibilities:  Informs employees of their rights and responsibilities under the Texas Workers' Compensation system.
• Notice to Healthcare Providers of Workers' Compensation Insurance Coverage:  Informs healthcare providers about the workers' compensation insurance coverage for employees, including details on medical care, billing, reimbursement, and provider responsibilities.
• Workers' Compensation Information:  Outlines the WCI program, detailing medical coverage, disability payments, and the process for reporting work-related injuries.
