ADA Packet of Information

The Americans with Disabilities Act (ADA) packet provides employees and healthcare providers with the necessary information to request reasonable accommodations for disabilities. This ensures compliance with ADA regulations and supports employees in performing their job duties effectively.

  1. Reasonable Accommodation Request Form: A form for employees to complete in order to request specific accommodations based on their disability.
     
  2. Medical Questionnaire for Reasonable Accommodations Request: To be completed by the healthcare provider to provide detailed information about the employee’s medical condition and recommended accommodations.
     
  3. Employee’s Position Description: This link will take you to log in to Workday to View Position Restrictions. In the View Position Restrictions dialog box, enter your UIN, press enter, and then select the OK button.  For a printable copy of the job description, select the PDF icon in the upper right-hand side below your profile photo, mark the position overview checkbox, and then print. This document describes the employee’s job duties and responsibilities, which helps the healthcare provider understand the physical and mental demands of the job.
Instructions for the Employee
  1. Review the Information: Understand your rights and responsibilities under the ADA.
     
  2. Consult Your Physician: Have your healthcare provider complete the Medical Questionnaire for Reasonable Accommodations Requests using your job description.
     
  3. Complete the Forms: Ensure all sections of both forms are accurately filled out.  The ADA and Other - Request for Accommodation Form will be completed by the employee.  The Medical Questionnaire for Reasonable Accommodations Requests will be completed by the health care provider.
     
  4. Submit the Request: Return the completed forms to your HR Professional promptly to avoid delays in processing.
