Access an Insurance Card

Overview

Use these resources to access an insurance card for your Medical, 65 Plus Medicare Advantage Plan, Express Scripts, Vision, Dental, and/or Academic Health Plans coverage.

Use this service request to ask questions about:

• Accessing Insurance Cards:  Learn how to access your insurance cards through your employment.

Coverage Start Dates

Choose when you want your coverage to begin.  Each option has a different enrollment deadline:

• Date of Hire / Initial Eligibility
  Enroll within 7 calendar days from your hire or eligibility date.
• First of Month Following Hire
  Enroll on or before the last day of the month in which you are hired or become eligible.
• Employer Contribution (SGIP) Date
  Coverage begins the first of the month following 60 days of employment. You must enroll within 31 calendar days from your hire or eligibility date.
