The Americans with Disabilities Act (ADA) packet provides employees and healthcare providers with the necessary information to request reasonable accommodations for disabilities. This ensures compliance with ADA regulations and supports employees in performing their job duties effectively.
- Reasonable Accommodation Request Form: A form for employees to complete in order to request specific accommodations based on their disability.
- Medical Questionnaire for Reasonable Accommodations Request: To be completed by the healthcare provider to provide detailed information about the employee’s medical condition and recommended accommodations.
